Investigation and Root Cause Analysis Training - Investigators' Master Class

The West Midlands Patient Safety Collaborative (WMPSC) is offering each organisation within the West Midlands region two delegate places on Investigation and Root Cause Analysis training courses being run in November and December 2017. These events will be free to delegates and each organisation may nominate a maximum of two delegate places for each training course. 

The objective of the Investigators' Master Class is to achieve enhanced clarity around specific areas of investigation practice, and to assist the audience in improving their investigation and report writing skills. It is only intended for staff who have significant experience of conducting serious incident investigations on behalf of their trust, including delivery of the investigation report. The trainer would like four trusts to work with it in the delivery of this workshop. Volunteer trusts would commit to briefly presenting a serious incident which resulted in in-depth investigation and RCA and a willingness to share the completed report (in advance) with fellow NHS colleagues attending the workshop.
